Tower Health is pleased to offer you information via a Web-based version of our computer systems called MyTowerHealth.

To request an account, submit a MyTowerHealth Consent Form to Tower Health, indicating that you have read this Terms & Conditions Statement.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

What is MyTowerHealth Medical Record Access?

MyTowerHealth Medical Record Access is an Internet application that enables a patient to have secure web-based access to portions of their electronic medical record, and allows a secure, convenient communications channel with Tower Health for non-urgent messages. Use of this service is optional. Your login name and password are key to protecting the confidentiality of your information. It is your responsibility to keep your password confidential. It is only known by you and can be changed online at any time by you. Your password is not known by anyone at Tower Health.

Response to Electronic Communication

Tower Health will make its best effort to provide a timely response to electronic inquiries. In some cases, the staff that needs to respond to an electronic inquiry or other communication may not be immediately available so a patient should allow one (1) to two (2) business days for a response. Accordingly, emergency situations requiring immediate attention should not be submitted electronically.

Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Web Site Links

MyTowerHealth may offer links to related medical Web sites not managed by Tower Health. These Web site link(s) are for a patient's informational purposes only. Tower Health does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.

Minor Eligibility

An individual must be age 18 or older to request a personal account. Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. Minor children between the ages of 14 and 17 (inclusive) may access their own online record with the consent of a parent or legal guardian.

Proxy Access

Birth parents or legal guardians may request proxy access to a minor's (under the age of 14) MyTowerHealth account by completing the Proxy Access portion of the Parental Authorization Form and submitting it to Tower Health. Such access will only be granted to parties with parental rights or legal guardianship over the MyTowerHealth account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder's medical information. A MyTowerHealth account will be activated for both the proxy and the account holder. If the proxy's legal relationship with the account holder changes, the proxy must inform the Tower Health immediately by calling the MyTowerHealth Helpdesk at 484-628-MYCH (6924) Monday through Friday from 8:00am to 5:00pm, or sending written notice to the MyTowerHealth Helpdesk at, RMS Suite 10, Reading Hospital, PO Box 16052, West Reading PA, 19612. Tower Health reserves the right to revoke proxy access at any time for any reason.

E-mail Privacy

Patients who are users of MyTowerHealth should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyTowerHealth. This means that any person with access to a patient's e-mail will be able to see this notification. This could include the patient's spouse, employer or anyone else that can access a patient's e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyTowerHealth may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.

Please know that if you send us an e-mail communication, it may be shared with the Tower Health staff that assists the physician in providing the patient's medical care. A patient's confidential medical information on MyTowerHealth will be accessible only to appropriate clinical staff.

Security and Confidentiality

We afford the same degree of confidentiality to medical information stored on MyTowerHealth as is given to medical information stored by Tower Health in any other medium. Tower Health is committed to protecting the confidentiality of your medical information. We limit Tower Health employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses MyTowerHealth. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

Secure Messaging Guidelines

Please observe the following guidelines for messages sent through MyTowerHealth:

  • Please DO NOT SHARE your password with anyone.
  • Please do not send any messages requiring urgent attention. Typical turnaround time for responses is one (1) to two (2) business days.
  • Please note that MyTowerHealth messages may be viewed by other health professionals and others who work with your physician.

Your Responsibilities

  • By accessing or using MyTowerHealth you agree to be bound by all the terms and conditions of this Agreement and the MyTowerHealth Privacy Statement as posted. We may modify this agreement and the privacy statement at any time, and you agree that such modifications are effective immediately upon posting of the modified version.
  • You agree that MyTowerHealth is a communication service offered as a convenience to patients. You understand that it provides you online access to portions of your medical record and that you are solely responsible for any sharing of its content that you intentionally or unintentionally communicate to others. You understand that messages will be routed to the appropriate department as necessary for handling, and therefore, employees of the Tower Health, other than your healthcare provider, may be involved in addressing your request. Therefore use of MyTowerHealth may not be appropriate for very sensitive information you wish to discuss directly with your provider.
  • For other than general information viewing, MyTowerHealth must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).
  • You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Tower Health by viewing the digital certificate. This certificate verifies the connection between the Tower Health server's public key and the server's identification.
  • User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyTowerHealth user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Time Considerations

MyTowerHealth should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is one (1) to two (2) business days. Therefore, for all urgent medical matters, please contact your physician's office by phone, go to an emergency room, or dial 911.

If your provider is out of the office or unavailable, messages sent via MyTowerHealth may be routed to other authorized caregivers within Tower Health in order to facilitate a timely response to your request.

Information Does Not Constitute Medical Advice

Tower Health assumes no responsibility for how you use the information you obtain from MyTowerHealth. You should consult with a physician or other health care provider regarding your own condition and how MyTowerHealth content may or may not apply to you.

You may be accessing your own medical information via the MyTowerHealth application and may wish to communicate with one of your healthcare providers about that or related information. If you have an emergency, or think you need to speak to someone urgently, please do not use the email communication options provided to you by MyTowerHealth. Neither MyTowerHealth nor any other application available through this site is a substitute for appropriate and timely contact with your physician. My Chart is not a substitute for consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.

Accuracy and Integrity of Information

Although Tower Health attempts to ensure the integrity of MyTowerHealth, it makes no guarantees whatsoever as to the correctness or accuracy of the site. It is possible that MyTowerHealth could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy arises, please inform Tower Health so that it can be corrected. Information contained on the site may be changed or updated without notice.

Termination of MyTowerHealth Medical Record Access

Tower Health may restrict or terminate access to MyTowerHealth at any time. If access is restricted for planned maintenance of the MyTowerHealth system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyTowerHealth without advance notice. If your use of MyTowerHealth violates these Terms and Conditions, Tower Health reserves the right to permanently terminate access to MyTowerHealth.

You can disenroll from your use of MyTowerHealth Medical Record Access at any time by completing the appropriate online request to disenroll or by calling your physician office. If we determine at our sole discretion that you have violated this agreement and/or abused the use of this service, we reserve the right to discontinue your use of this service. You will be notified if we discontinue your MyTowerHealth Medical Record Access service.


This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.
